Referral for coronary artery calcium (CAC) scoring where appropriate.

Chronic kidney disease management focusing on halting disease progression and preserving kidney function. Aggressive control of reversible risk factors including high blood pressure, obesity and diabetes.
Utilization of targeted renal therapies to improve long term kidney and cardiac outcomes.
Specialized blood and urine testing is used to accurately monitor kidney function and response to treatment.
Proactive Care Coordination including timely referrals to a Kidney Care Clinic or dialysis orientation, if needed.
